Local Funds
Please see below for a summary of local services used by local funds.
Family Support Services
Family Support Services are intended to provide assistance or additional support to families and people served and are designed to fill gaps or address exceptional needs related to a person’s diagnosed disability. Requests for Family Support Services will be discussed during the annual service plan meeting and are allotted per household.
Community Connections
Community Inclusion funding promotes a person’s full participation in their community. Requests should incorporate and promote inclusive community membership. Requests for Community Connections funds must be discussed with your Service and Support Administrator (SSA).
Supported Living
Supported Living refers to funds that can help you live and thrive in your community. Your Service and Support Administrator can help to determine if Supported Living is appropriate to best meet your needs.
Transportation
Transportation funds are available, on a limited basis, to help you get where you need to go. Access to these transportation funds is based on your age and unique needs. You Service and Support Administrator can help to determine if transportation can best meet your needs.
Adult Services
Adult Services refers to a range of options to participate in programming specific to people with developmental disabilities. The array of adult services that may be funded is determined by the needs of the person and is included in your formal service plan.
